The card is valid from September 1st to July 31st of each academic year. <\/p>\n<p>In order to obtain the University Sports Center membership card, students must submit the following documents:<\/p>\n<ul>\n<li>&nbsp;Application form\u2013 paper version available at the University Sports Center Secretariat, 1<sup>st <\/sup>floor, 09:00-13:00 (can also be sent via email)<\/li>\n<li>Two photos<\/li>\n<li>Medical certificate of aptitude for sports activity (valid for one calendar year) signed by a personal physician (general practitioner, cardiologist) or public healthcare provider (hospital, EOPY, etc.). Students who are male and female athletes can provide a copy of the health certificate from their sports club. <\/li>\n<li>Solemn declaration \u2013 available both in electronic and paper format (at the University Sports Center Sectetariat)<\/li>\n<li>Student ID or recent Certificate of Study (to be shown only)<\/li>\n<\/ul>\n<p>The membership card is issued within one to three working days.
